	Kodak DC240 Software CD-ROM (Windows 95, 98 or NT)		Kodak DC240 Software CD-ROM (Mac System 7.5.5/iMac System 8.5)

I expect to return the camera by         /        /20        .

If borrowing for more than 7days[1], please state the reason for borrowing camera,  e.g. preparing for lecture, seminar, or presentation. Include details like title, event, date. _____________________________________________________________________

If I have borrowed either one or both of the CD-ROMs, I understand and fully agree that,

1.      I may install the Kodak Software (Mounter, TWAIN Acquire, Camera Controls and Border Transfer Application) for use only with Kodak cameras, and

2.      I shall NOT install the Picture Easy Software found on the above CD-ROM’s.

If the camera is lost, I promise to pay back its current market value (as decided by the Doctor-in-charge).

I have read and fully understand the Rules and Regulations pertaining to the use of this camera and its accessories and hereby agree to be bound by its terms.

Permission is hereby granted to the applicant to borrow the above items up to the date quoted above. (Only required if borrowing for more than 14 days[2].)
